Analisis Perbandingan Luasan Sadapan ST-Elevasi Precordial dengan dan tanpa High Lateral terhadap Fungsi Ventrikel Kiri pada Pasien Late Onset-STEMI

Pendahuluan: Infark miokard akut dengan elevasi segmen ST (STEMI) pada pasien yang datang terlambat (late onset-STEMI) berisiko mengalami penurunan fungsi ventrikel kiri. Pemeriksaan EKG merupakan alat utama untuk menilai distribusi ST-elevasi sebagai indikator luas infark. Studi ini menilai apakah keterlibatan high lateral lead (I dan aVL) pada ST-elevasi precordial berkaitan dengan penurunan fraksi ejeksi ventrikel kiri (LVEF ≤35%).
Metode: Penelitian ini merupakan studi observasional analitik dengan desain potong lintang pada 35 pasien STEMI anterior late onset di RS UNS dan RSUD Dr. Moewardi. Subjek diklasifikasikan berdasarkan distribusi sadapan ST-elevasi: hanya precordial lead dan precordial + high lateral. Variabel utama adalah penurunan LVEF ≤35%. Analisis statistik menggunakan uji Chi-Square, uji t independen, dan Linear-by-Linear Association.
Hasil: Hanya variabel chronic kidney disease (CKD) yang memiliki hubungan signifikan dengan penurunan EF ≤35% (p=0.034, r=0.348). Variabel lain seperti usia, jenis kelamin, hipertensi, diabetes melitus tipe 2, dislipidemia, stroke, merokok, troponin, dan onset tidak menunjukkan hubungan bermakna. Perluasan ST-elevasi ke high lateral lead juga tidak menunjukkan hubungan signifikan terhadap penurunan EF ≤35% (p=0.404), dengan odds ratio 1.820 (CI95%: 0.443–7.477).
Kesimpulan: Keterlibatan high lateral lead pada pasien late onset-STEMI dengan ST-elevasi di precordial lead tidak berhubungan signifikan dengan penurunan EF ≤35%. CKD merupakan satu-satunya faktor yang secara signifikan berkorelasi dengan penurunan fungsi ventrikel kiri yang berat pada poulasi penelitian.
